Push and pull factors to the involvement of young female physicians in medical academic careers: global study and focus on gender issues.

Many studies show that interest for academic careers in medicine is declining, the most important sign of the poor state of academic medicine being a fall in recruitment to clinical academic posts in Europe during the past decade (1,2). Given international concern about the need to revitalize academic medicine and its leadership, a better understanding of the attractiveness and the obstacles for choosing a medical career is needed.
At the same time as the academic decline, the feminization of the medical profession is continuously increasing worldwide and is now largely predominant in France (3). Women have been entering medical school in larger numbers as men for decades (60% in France, (4)). In marked contrast, women remain underrepresented in academia as much in France as at the international level (5).
The disadvantages of fewer women choosing careers in academic medicine include a waste of intellectual capital, and a potential lack of diversity in the research agenda and future health practices(6–8). The Lancet fully dedicated a full issue to these questions (9).
We hypothesize that the decline in medical academia may be in part due to an insufficient recruitment of women, and that the ideal mentoring/guidance/selection process of young female physicians may not be identical to the one ideal for young male physicians.
The aims of this project are to assess the respective contribution of the determinants which contribute to academic decline and to gender gap in medical academic careers, and to articulate these variables within a global comprehensive model accounting for gender specific effect.
